Who We Are

 On October 31, 2013, Fauquier Health and LifePoint Hospitals formed a joint venture to share ownership and operation of Fauquier Health, funding a charitable foundation to support crucial community needs. 

The PATH Foundation (originally known as the Fauquier Health Foundation) Board of Directors convened for the first time on November 20, 2013. Meetings were regularly held to determine the mission, vision, strategic plan, funding priorities and grant making criteria. 

To inform the process, Fauquier Health and the PATH Foundation partnered to conduct a community health needs assessment to identify existing health priorities, note service gaps and determine where the foundation can turn its focus to support the health and vitality of the community. The PATH Foundation is committed to honoring the legacy of community philanthropy by being good stewards of the foundation's assets and by using these assets to stimulate community benefit, inspire citizen engagement and support area resources. Our goal is to enhance the region as a place where everyone has the opportunity to have a healthy place to live, work, play and grow. 

With a legacy of community philanthropy stretching back to 1954, the modern PATH Foundation manages the assets of this multigenerational gift to promote healthy communities today and in stewardship for our children’s grandchildren

What We Do

The PATH Foundation strives to enhance the health and vitality of our community.

By the year 2025, The Path Foundation's footprint will lead the state in overall health status improvement and private capital investment, with the goal of a 20% rise in Robert Wood Johnson Foundation County Health Rankings for each of our three counties.
