Trusted Riders Chaperones help people get to and from the places they need and want to go, ensuring the client's comfort and security with a helping hand throughout the journey and making ANY form of available transportation ACCESSIBLE. They assist clients throughout their travel and, if necessary, at their destination. They are valuable in helping clients needing essential services and prioritizing non-emergency medical care.
What You’ll Do:
· Provide door-to/through-door service to and from doctors’ appointments, medical procedures, and other essential services and serve as the “responsible adult” for discharge required by providers and government and private insurance;
· Help patients with facilitating scheduling of rides to or from appointments in coordination with RaH staff;
· Converse with patients about how they are feeling upon arrival at the door, and will report a situation that needs immediate action to appropriate support for assistance in a time period often quicker than a post-procedure check-in call from a nurse;
· Phone check-ins with patients’ hours and day after the trip home; and
· Additional wellness checks for individuals who may have recently been ill, had a medical procedure, or feel disconnected from family, friends and neighbors in coordination with RaH staff.
Why It Matters: Help people ensure they get to their appointments, record what the doctors tell them, make follow up appointments in support of their health and wellness.
Commitment:Trusted Rider volunteers must participate in trainings including CPR and first aid and undergo a background check. All trainings and background checks will be provided by RaH.
